What are the parts of a Dazzle capture card?

Hey, about 8-12 months ago, I bought a Dazzle capture card. It cost me around $40 to $50. I didn't really use it, so I stuffed it away in a box full of cables, computer accessories, xbox accessories, etc. Now, I have a need for it, but I cannot put it together. I have all the parts, I just have to separate them, and I can't tell which ones are for my Dazzle. Can someone help me?? Is there a link that tells you how to put a Dazzle capture card together??
